This document serves as an agreement between FWJ Inc. d/b/a InitiativeOne ("InitiativeOne") and the undersigned individual and company ("Client") for the delivery and completion of the following services.

PDP ProScan® One-on-One Consultation with an InitiativeOne Facilitator
Leadership Transformation Notebook
MP3 Chapter Recordings
Thirteen (13) Three-Hour Leadership Transformation Sessions
Eligible for 16.8 CEUs through UW-Green Bay.
INVESTMENT
$6,900 per participant
CONSENT & AUTHORIZATION
I consent to InitiativeOne surveying participants utilizing InitiativeOne services and using data collected anonymously for academic research.
I give InitiativeOne consent to include my company's name and logo on InitiativeOne's Client page.
